D-95 is Distiller #95 which is an identifier for the company with this permit # Unfortunately I have no info on this particular permit #
As for the patent without a number or other info, this will be difficult work. I have been assembling a database of bottle related patents for years, but unfortunately I have not yet posted it online.
Do post a clear photo of the entire bottle since there is always a chance I might recognize the bottle by shape alone.
